Terms and conditions

Golden Valley Mobile Imaging LLC


1. Company Overview

Golden Valley Mobile Imaging LLC provides mobile diagnostic imaging services throughout the State of California to skilled nursing facilities, assisted living communities, hospice agencies, and other approved healthcare environments. By requesting or utilizing services, the requesting facility or authorized representative agrees to these Terms and Conditions in full.


2. Scope of Services

The Company provides mobile digital X-ray services and may provide add-on EKG services when ordered in conjunction with imaging studies. Services are performed onsite at the requesting facility. All diagnostic images are securely transmitted to a contracted, licensed radiologist for interpretation. Golden Valley Mobile Imaging LLC does not diagnose, prescribe, or make treatment decisions. Final interpretation and reporting are the responsibility of the radiologist.


3. Physician Order Requirement

All services require a valid and properly authorized physician or licensed provider order prior to performance. Orders must include accurate patient demographic information, the specific examination requested, clinical indication or medical necessity, and the ordering provider’s name and signature. The Company reserves the right to delay or decline service if required documentation is incomplete, unclear, or inconsistent with regulatory standards.


4. Facility Responsibilities

The requesting facility is responsible for ensuring safe and reasonable access to the patient, providing accurate demographic and clinical information, confirming medical necessity, and maintaining a safe working environment. The Company may decline or suspend services if working conditions are unsafe, if patient access is not properly prepared, or if documentation requirements are not met.


5. Turnaround Times

Estimated turnaround times for routine and STAT examinations are provided as general guidelines and may vary based on clinical volume, technical considerations, transmission connectivity, or circumstances beyond the Company’s control. While reasonable efforts are made to prioritize urgent examinations appropriately, specific reporting time guarantees are not implied unless otherwise agreed to in writing.


6. Billing and Payment Terms

During the initial phase of operations, services may be billed directly to the requesting facility in accordance with agreed pricing and payment terms. Standard payment terms are Net 15 unless otherwise specified in writing. Late balances may be subject to administrative fees. As the Company transitions to insurance billing, claims may be submitted through a third-party billing service. Reimbursement from Medicare, Medi-Cal, or private insurance carriers is not guaranteed, and the requesting facility or responsible party may remain liable for denied, unpaid, or partially reimbursed balances.


7. Pricing Acceptance

Pricing must be confirmed prior to service unless a standing agreement exists between the Company and the requesting facility. Submission of an order constitutes acknowledgment and acceptance of the Company’s current pricing structure. The Company reserves the right to update pricing with appropriate notice.


8. Cancellations and Service Limitations

Same-day cancellations, patient unavailability upon arrival, incomplete documentation, or circumstances preventing service delivery may result in applicable service charges. The Company also reserves the right to decline service in situations that pose safety risks or regulatory concerns.


9. Radiology Services

All diagnostic images are transmitted to a contracted, licensed radiologist for interpretation. The radiologist operates independently and is solely responsible for final diagnostic reporting. Golden Valley Mobile Imaging LLC is responsible for image acquisition and transmission but does not assume responsibility for medical diagnosis.


10. Regulatory and Safety Compliance

Golden Valley Mobile Imaging LLC operates in compliance with applicable federal and California state regulations, including those administered by the California Department of Public Health Radiologic Health Branch. Equipment is maintained and operated according to established radiation safety standards, and ALARA principles are followed at all times.


11. Privacy and HIPAA Compliance

The Company complies with applicable HIPAA regulations and safeguards Protected Health Information in accordance with federal and state privacy laws. Patient information is transmitted and stored using secure systems designed to protect confidentiality.


12. Limitation of Liability and Indemnification

To the fullest extent permitted by law, the Company shall not be liable for clinical decisions, treatment outcomes, or patient management actions taken by the requesting facility or its providers. The Company’s liability, if any, shall be limited to the amount charged for the specific service rendered. The requesting facility agrees to indemnify and hold harmless Golden Valley Mobile Imaging LLC from claims arising out of inaccurate patient information, unsafe working conditions, misuse of imaging results, or other circumstances beyond the Company’s direct control.


13. Governing Law

These Terms and Conditions are governed by the laws of the State of California. Any disputes arising from services rendered shall be resolved within the State of California.


14. Modifications

The Company reserves the right to update or modify these Terms and Conditions at any time. Continued use of services constitutes acceptance of the most current version.
